I like the way TS3 did it where the mom would go into the hospital, dad or someone would go into be with her and it all happens behind the scenes.
I remember one time in TS3 my sim was dating this guy (they did not live together) and she got pregnant. When it came time to give birth, I sent her to the hospital and he automatically appeared shortly after carrying a toddler. I found that a little weird so I investigated a little and it turned out he was already living with (but not married) to another sim and that was their toddler. Oh, the drama after that. I loved it!
